Before shipping ANY equipment to MDL, a “Return Material Authorisation” (RMA) number is needed and assigned to the customer. The following information is required:
- Company name, billing and shipping addresses, telephone, fax number, and contact name
- Part number and serial number.
- Credit Card details or Purchase Order if the customer has an authorised credit account.
- Reason for return: problems, symptoms.
- Any special handling instructions.
IMPORTANT After information has been taken, the RMA number will be generated. The RMA number needs to appear on top of the box and on any paperwork sent to the Service and Repair Centre. Please also include the approved RMA Log Form.
